CANCELLATION OF APPOINTMENTS

New Patient Appointments: We require a credit card number to secure your new patient visit. In the event that you fail to show up on time or cancel with less than 24 hours notice you will be charged a $50.00 fee. This fee must be paid before a new appointment is scheduled. Patients with two (2) consecutive missed appointments without giving adequate notice will be asked to seek treatment with another physician.

Established Patient Appointments: Patients who do not show up on time for an appointment, or cancel with less than 24 hours notice will be charged a $30.00 fee. This fee must be paid before a new appointment is scheduled. Patients with three (3) consecutive  missed appointments, without giving adequate notice will be asked to transfer their records to another physician.

Cosmetic and Surgical Appointments:

We set aside special allotted time for surgical and cosmetic patients and do not overbook these appointments. We also would like to be able to accommodate our patients and see them as soon as possible. For these reasons, we require a credit card to hold your surgical appointment and a deposit of $125.00 for cosmetic appointments (which will go towards your cosmetic procedure). If you fail to cancel your scheduled time within 48 hours your credit card will be charged a fee of $125.00 for surgical appointments or you will forfeit your deposit for your cosmetic procedure. All Medspa packages will be charged a cancellation fee of $300.00 plus cost of  any additional medication or devices or garment that must be purchased prior to the procedure.
